  • A qualifying physician certifies appropriate patients in the office to receive a medical card to buy products off-site at a number of state-approved dispensaries, under the guidance of the qualifying physician.

  • The current medical conditions for Florida include:

    Cancer

    Epilepsy

    Glaucoma

    HIV/AIDS

    Post-traumatic stress disorder (PTSD)

    Amyotrophic lateral sclerosis (ALS)

    Crohn’s disease

    Parkinson’s disease

    Multiple sclerosis (MS)

    Other onditions related to muscle spasms, migraines, osteoarthritis, rheumatoid arthritis, fibromyalgia, neuropathy, digestive diseases, high blood pressure, restless legs syndrome and Sjogren’s syndrome.

    Medical condition of the same kind or class as or comparable to the others listed. A terminal condition diagnosed by a physician other than the qualified physician issuing the physician certification.

    Chronic non-malignant pain caused by a qualifying condition or that originates from a qualifying condition and persists beyond the usual course of that qualifying medical condition.

  • No. A primary care physician referral is not required.

    1. Initial Consultation - Medical Evaluation: The first appointment will be a dedicated one-hour View ratessession with Dr. Schramm who will discuss your medical history and goals. View rates.

    2. State Registration Fee: The Florida Department of Health charges $75 annually for a medical card.

    3. Renewal Fees: Florida medical cards are valid for one year. To maintain your card's validity, simply schedule a follow-up consultation (every seven months) at Vibrant Health.

  • At this time, Insurance companies and Medicare do not pay for medical cards, office visits or products.

  • You will need to provide a state drivers license or ID card, social security number, e-mail address, home address, and medical records. You will be asked to sign a standard consent form. All certification visits are conducted in the office by a qualifying physician.

    Once qualified, you will receive an email inviting you to log into the OMMU site to register as a patient and pay a $75 fee, which is good for 1 year. Once completed, you are then eligible to proceed to the dispensary of your choosing to purchase your product. This can often be completed the same day.

  • You will need to provide medical records that document your medical diagnoses, prescription medicines, and allergies. These do not necessarily need to be extensive and may include a chart summary or a clinical appointment summary. Provisions for medical records release from your physician can be made in advance of your appointment.

  • Yes. One of the most appealing aspects of natural therapy, is the prospect of weaning from or avoiding certain classes of medications, including opiates for pain, benzodiazepines for stress and anxiety, and sedative hypnotics that are often prescribed for sleep. In addition, patients suffering from pain, who cannot take anti-inflammatories (NSAIDS) or aspirin or on blood-thinners or find inadequate relief from Tylenol or other over-the-counter pain relievers are ideal candidates. It is important for you to consult with your primary care doctor, pain specialist or psychiatrist before stopping any medication.
